Step 1: Go to FACTS SIGN IN and create a username and password. Complete the FACTS application online (cost $40). Please upload the supporting documents to the FACTS website.


Supporting documents include:


  • Copies of your most recent Federal tax forms including all supporting tax schedules
  • Copies of last years W-2 forms for both parents
  • Copies of supporting documentation for Social Security Income, Welfare, Child Support, Food Stamps, Worker's Compensation, and TANF.
     

Step 2: If your family income falls within the chart below then please complete and sign the 3 page Catholic Education Foundation. CLICK HERE for CEF Form (English).  CLICK HERE for CEF Form (Spanish). Go to cefdn.org for further instructions. If your family gross income is higher than the chart below indicates, then you only need to complete Step 1.

CEF defines a household as all individuals living together in the same dwelling who share expenses for rent, utilities, food, clothing, and other necessities. A one-member household is a student who is his/her sole support, such as an institutionalized minor or adult. A foster child is only considered a one-member household if the welfare/placement agency maintains legal responsibility for the child.
